Case 1

Rick McHale is a 58 year old male coming in for a follow up visit.  He was seen two weeks ago for a physical by your partner who is now on maternity leave. He is here today, as instructed, for his lab results. He has a strong family history of coronary artery disease as his father died of a heart attack and his older brother had quadruple bypass.

Review below with Mr. McHale and discuss your concerns about his health.

Case 2

Take this opportunity to review vitals/ labs and to talk to Mr. Walker about your concerns about his health.
